Abstract | The number of patients with severe motor and intellectual disability (SMID) has continued to increase. Our previous study on the utilization of welfare systems by patients with SMID and their parents in 2014 found that the utilization of home-visit medical care, home nursing, day care, and respite care remained low, whereas the requirement for the same services were high. The current study therefore aimed to examine changes in the utilization of welfare systems by patients with SMID and their parents over the past few years and determine satisfaction and problems when living at home. Caregivers of 33 patients with SMID who visited the outpatient clinic of Tottori University Hospital in 2018 were interviewed according to a structured questionnaire. Accordingly, our results showed that the utilization rates of home-visit medical care, home nursing, day care, respite care, and home-visit rehabilitation in 2018 was greater than those in 2014. While levels of satisfaction were generally high in all services, several problems had been encountered while supporting the daily life of patients with SMID and their families at home. The problems included inadequate cooperation, limited facilities, insufficient staff, and inadequate welfare system. As such, enhancing facilities, cooperation between facilities, and the welfare system is imperative.
