During a brain tumor biopsy procedure, the patient suffered cardiac arrest from massive bleeding and was admitted to the intensive care unit（ICU）after resuscitation. Dexamethasone was administered daily as treatment for the brain tumor starting on postoperative day（POD）1. Her general condition gradually improved and she was discharged from the ICU on POD11. She was readmitted to the ICU due to hemorrhagic shock on POD15. Gastrointestinal endoscopy showed duodenal ulcer bleeding, which was stopped by endoscopic clipping. Gastrointestinal bleeding again caused hemorrhagic shock on POD17 and 20, which was stopped by transcatheter arterial embolization. The test results for CMV antigenemia were positive on POD25. Histopathology revealed nuclear inclusions characteristic of CMV enteritis and ganciclovir therapy was initiated on POD26. She was discharged from the ICU on POD38. The duodenal ulcer improved and she was discharged from the hospital on POD109. When refractory gastrointestinal bleeding is observed, CMV infection should be considered in the differential
