| 内容記述 | The maldistribution of physicians in Japan has been assessed through a standardized index introduced by the Ministry of Health, Labour, and Welfare in 2019, which incorporates multiple demographic and geographic factors. However, regional disparities and unique challenges remain inadequately addressed, including the lack of young physicians and poor healthcare access in rural areas. This study proposes an adjusted physician maldistribution index that incorporates three additional parameters: resident physician matching rates and indicators of medical accessibility, such as public transport share and proximity to medical facilities. Correlation analyses between these parameters and health outcomes, represented by age-adjusted mortality rates, were conducted across Japan’s prefectures. The results showed that prefectures with high accessibility and residency matching rates ranked higher when using the adjusted index. Conversely, rural regions with low residency matching rates and limited medical accessibility experienced a decline in ranking. The proposed adjusted index provides a more accurate representation of physician distribution, addressing region-specific challenges and informing targeted policy interventions. | |||||||||
